MyCareScope Disability and Community Services

Whether you are getting your annual check-up or any health appointments, or going to work, or simply looking to go out on an activity of your choice. MyCareScope will provide you with an attendant care staff that would make your attendant and community access outing all about you. We encourage participants to pursue their interest and will join you in your activity of choice. We will help source out for group activities that interests you and will join you for motivational support if required. 

MyCareScope is aimed at promotion of Social Inclusion as part of model of care. We make it all about you and your care. We assist and encourage you to build on your threshold/capacity and social life. We believe having an active social life of your choosing is healthy for your mental and emotional health and helps in the development of your confidence and builds more on your capacity to do things Independently. Social Inclusion is also captured in your Independent-Scoped Care-Plan which is continually updated for your needs.
