MyCareScope Disability and Community Nursing Services

MyCareScope supports your In and Out of Home activities. MyCareScope will support you in your day to day activities whilst at home or Out of the house. We offer Attendant Care as well as Community care and Sleepovers. MyCareScope offers in-home Respite care and care supervision. With consent, we help create an Independent-Scoped Care-Plan based on your NDIS Plan and Goals, Ensuring your care is Independently all about you. We encourage Social Inclusion and advocate for pursuing of interests, hobbies and Independent community engagement. We will assist with your house chores if needed, including domestic duties, shopping, personal hygiene and your general fundamental needs. MyCareScope works closely with clients, and encourages inclusion in decision making, while using clients Independent-Scoped Care-plan to achieve goal of care.
